Information |

| According
to Florida law, a parking permit may be issued to anyone with disabilities that limit or impair the person’s ability to
walk or is certified as legally blind. |

The Permanent Parking Permit (blue) is
issued for up to four years from the date of
issuance and expires on the applicant’s birthday. Renewals are for the full four years. An additional permit may be issued
if you qualify as a frequent traveler or as a quadriplegic.
There is no fee for a permanent parking permit but a Florida driver's license or
Florida ID card is required. |

The Temporary Parking Permit (red) is
issued for a period not exceeding 6 months. The cost is $15.00. If necessary, a second permit may be issued when the
first permit expires with another application form completed by the
physician. If done within a twelve-month period from the date the first
permit was issued, the second permit will be issued for no fee. A Florida
driver's license or Florida ID card is also required. |

How do I apply for a permit?
Complete an application form (HSMV 83039)
and have it certified by a licensed
physician, osteopath, podiatrist, chiropractor, optometrist, a physician who
practices medicine in a military medical facility, state hospital or federal
prison, advanced registered nurse practitioner or physician assistant.Mail the application (except for out-of-country visitors) and fee (if

How do I renew my permit?
Permanent parking permits must be renewed by your birthday every four years . There is no cost to renew. Although
there is no cost, you must still advise us of your eligibility to have the
permit(s), by using one of two easy
ways - over the internet or by phone. When we have received your
renewal information, a new permit will be mailed to you. Please allow
two weeks for processing.

recording. You will need to have your permit number(s) or your
renewal postcard available when you call.

Parking Permits for Organizations
|
Permits may be issued to Florida organizations that provide regular
transportation services to disabled persons having disabilities that limit
or impair their ability to walk or are legally blind. Parking permits
are issued for four years and expire June 30.

| The Sunshine State plate may be issued to a disabled person who owns a
vehicle and qualifies for a disabled parking permit. The DV wheelchair
plate is issued to a disabled veteran who has a DV license plate and
qualifies for a parking permit. Both plates entitle the vehicle to
park in any designated disabled parking space. |

Out-of-State Visitors
Florida has an agreement with every state which allows out-of-state
permits to be used in Florida. These states also recognize Florida
permits along with all Canadian Provinces and Territories except Northwest
Territories and Nunavut. |

Out-of-Country Visitors
Visitors from other countries who have valid disabled parking permits from their resident
country may purchase a temporary permit for $15.00. This permit is
valid for 6 months. They must show the valid
out-of-country permit and a passport (or other comparable identification).
This permit cannot be issued through the mail. Original documents and
identification may be brought to any of our office locations. The documents may be brought in by a
family member, friend or guardian and will be returned. |
